You can call DHHR at  1-866-545-6502 and request a new EBT card. If you have a new address, you must call your local office Worker first to give them your new address so DHHR can mail you a new EBT card.

If you are the Benefit Payee, you must have your birth date and Social Security Number. If you are an Authorized Cardholder (AR), you must have the Birth Date of the Benefit Payee.

Call 1-866-545-6502 (the EBT Hotline). Press the number 1 on the telephone keypad for directions in English or press the number 2 for directions in Spanish.

• DHHR will ask for your 16-digit card number. If you have your number, you should enter it.
• If you do not have your card and do not know your number, do not do anything when DHHR asks you for your 16 digit number
• DHHR will tell you what number to press on the telephone keypad to report your card lost or stolen.
• To request a new card, you will need to provide the birthdate of the Benefit Payee cardholder. You must give the birth date of the Benefit Payee even if you are an Authorized Cardholder.
• When DHHR asks you for the Social Security number, if you are the Benefit Payee Cardholder you will enters your own SSN. If you are an Authorized Cardholder you will enter nine zeros.
• You should receive a new card within 5-7 days of your request.

There is no time limit or specific time frame in which you must request a replacement check. You must go to your local DHHR and meet with a Worker who will help you fill out a Lost Check Affidavit form. Your form will be sent to the DHHR State Office in Charleston for review, and if it is approved you will get a replacement for your ruined or lost check.
