Online: Online Application
Our Online Application is closed through Wednesday, January 1, 2025.
Please note: When you apply online, you will still need to provide a phone number where you can be reached so we can speak with you directly. Before applying, you may want to review our eligibility guidelines.
Things to Remember When Applying Online:
- Please DO NOT fill out more than one application for the same legal issue.
- We will call you FOUR times to try to reach you. If you miss the first three calls, you do not need to call us back. We will call you again. If you miss the fourth call, we will send you a letter with information about how to call us back.
- If you have a hearing or deadline within the next 10 days, DO NOT use the Online Application. Instead please call our Application Line at 866-255-4370 or reach out to your local office.
NOTE: Our Long-Term Care Ombudsman Program has a separate application process. To apply for their services, click here for additional information and contacts.
